Key Responsibilities
? Conduct home visits and perform comprehensive social work assessments, counseling, case management, and crisis intervention.
? Co-facilitate support groups, offering a safe space for patients and their caregivers.
? Organize and coordinate monthly educational programs to empower and inform stakeholders.
? Engage in chapter-wide events to foster community connection and outreach.

Required Skills
? Active New York State LSW license.
? Minimum of three years of relevant experience in social work, with an emphasis on end-of-life care.
? Exceptional communication and interpersonal skills.
? Ability to work autonomously as well as collaboratively within a team environment.
? Bilingual capabilities are advantageous.
? Valid driver?s license and access to a vehicle.

Qualifications
? Licensed Social Worker (LSW) with a valid New York State license.
? At least three years of direct experience in a similar capacity.
? Demonstrated knowledge and experience in end-of-life care settings.
