What To Expect (Job Responsibilities)
? Engage with patients to navigate their care and address healthcare issues
? Collaborate with medical practice staff and the Case Management team
? Document patients' medical history and issues in the electronic health record (EHR)
? Implement and meet assigned Key Performance Indicators to improve patient outcomes
? Utilize various communication methods to engage high-risk patients

What Is Required (Qualifications)
? Certified or Registered Medical Assistant OR certified EMT
? Experience with medical terminology and understanding of primary care physician practices
? Proficiency in using electronic health records (EHRs) and care management analytics
? Exceptional written, visual, and verbal communication skills
? Ability to work independently as part of a patient's care team under supervision

How To Stand Out (Preferred Qualifications)
? Familiarity with electronic health records such as EPIC, Athena, or Cerner
? Experience in motivational interviewing and behavioral change management
? Strong critical listening skills and empathy in patient coordination
? Ability to manage multiple tasks while remaining detail-oriented
? Proven success in a remote work environment
