About the position

We Care Home Health is seeking a compassionate and skilled Licensed Practical Nurse (LPN) to provide specialized care for an extended care patient with trach, vent, and G-tube needs. This role involves working night shifts in the patient's home, ensuring their safety, comfort, and medical stability. The position offers both part-time and full-time opportunities, allowing for flexibility in scheduling while delivering meaningful, one-on-one patient care.

Responsibilities
? Delivering specialized care for a patient with trach, vent, and G-tube.

? Monitoring the patient's respiratory and nutritional health during shifts.

? Administering medications and tube feedings.
,
? Performing routine assessments, interventions, and adjustments as necessary.

? Communicating with the care team and maintaining accurate patient records.

? Developing strong, supportive relationships with the patient and their family.

Requirements
? Current Colorado LPN license.

? Experience with trach, vent, and G-tube care preferred.

? Current BLS/CPR certification.
,
? Strong clinical and communication skills.

? Ability to work independently in a home care setting.

? A compassionate, patient-focused approach to nursing.
