Requirements

1. Valid and active Registered Nurse (RN) license.

5. Proficiency in using electronic health records (EHR) systems.

6. Ability to work independently and manage time effectively.

7. Reliable internet connection and a quiet, dedicated workspace.

Responsibilities

1. Conduct telephonic assessments and provide clinical guidance to patients.

2. Develop and implement individualized care plans.

3. Monitor patients’ progress and adjust care plans as necessary.

4. Educate patients about disease management, prevention, and treatment options.

5. Collaborate with other healthcare providers to ensure comprehensive care.

6. Document all patient interactions accurately in the EHR system.

7. Participate in ongoing training and professional development opportunities.

Educational Qualifications

– Bachelor of Science in Nursing (BSN) preferred but not required.

Experience
