Job Description

Requirements

– Current, unrestricted RN license in the state of residence

– Minimum of 3 years of clinical nursing experience

– Proficient in telehealth technologies and electronic health records (EHR)

– Exceptional communication and interpersonal skills

– Ability to work independently and as part of a team

– Strong organizational and time management skills

– Commitment to maintaining patient confidentiality and privacy

Responsibilities

– Conduct virtual patient assessments and triage care needs

– Develop and implement personalized care plans

– Educate patients and their families on disease management, medication adherence, and lifestyle changes

– Collaborate with healthcare providers to coordinate patient care

– Monitor patient progress and adjust care plans as necessary

– Provide emotional support and counseling to patients and families

– Maintain accurate and up-to-date patient records in the EHR system

Educational Qualifications

– Bachelor of Science in Nursing (BSN) required; Master of Science in Nursing (MSN) preferred

Experience
