Job Description

CVS Health is seeking an experienced and passionate Remote Diabetes Nurse Educator to join our dedicated team. In this role, you will provide essential diabetes management education and support to our patients through telehealth platforms. You will empower patients to achieve optimal diabetes control and improve their overall quality of life. Requirements

• Valid RN license in the state of residence.


• Certified Diabetes Educator (CDE) or Board Certified-Advanced Diabetes Management (BC-ADM) preferred.


• Minimum of 3 years of experience in diabetes education and management.


• Proficiency in telehealth platforms and electronic health records (EHR).


• Excellent communication, interpersonal, and organizational skills.


• Ability to work independently and collaboratively within a team environment.


• Strong problem-solving skills and attention to detail.

Responsibilities

• Provide comprehensive diabetes education to patients via telehealth platforms.


• Assess patient needs and develop personalized diabetes management plans.


• Monitor patient progress and adjust care plans as needed.


• Collaborate with healthcare professionals to ensure coordinated care.


• Document patient interactions and maintain accurate health records.


• Stay up-to-date with the latest diabetes management guidelines and best practices.


• Participate in ongoing training and professional development activities.

Educational Qualifications

• Bachelor’s degree in Nursing (BSN) required, Master’s degree preferred.

Experience

• Minimum of 3 years of experience in diabetes education and management.
