Job Description
CVS Health is seeking a dedicated and compassionate Remote Telehealth Case Manager to join our growing team. As a vital part of our telehealth services, you will play a crucial role in providing high-quality care to our patients from the comfort of your own home. As a Remote Telehealth Case Manager, you will be responsible for managing a caseload of patients, coordinating care, and ensuring that all health-related needs are met. You will work closely with patients, their families, and other healthcare professionals to provide comprehensive care plans and support. Your role will involve assessing patient needs, developing care plans, providing education and resources, and monitoring patient progress.
Requirements
1. Bachelor’s degree in Nursing, Social Work, or a related field.
2. Current, unrestricted RN or LMSW license in the state of residence.
3. Minimum of 2 years of clinical or case management experience.
4. Strong understanding of telehealth services and technology.
5. Excellent communication and interpersonal skills.
6. Ability to work independently and manage time effectively.
7. Proficient with electronic medical records (EMR) and other healthcare software.
Responsibilities
1. Manage a caseload of patients through telehealth platforms, providing comprehensive case management services.
2. Conduct thorough assessments of patient needs and develop individualized care plans.
3. Coordinate care and services with other healthcare professionals, including doctors, nurses, and therapists.
4. Provide education and resources to patients and their families to promote understanding and self-management of health conditions.
5. Monitor patient progress and adjust care plans as necessary to achieve optimal health outcomes.
6. Maintain accurate and up-to-date patient records in compliance with HIPAA and other regulatory standards.
7. Participate in ongoing training and professional development to stay current with best practices in telehealth and case management.

Educational Qualifications
