Requirements
1. Active RN license in the state of residence.
2. Minimum of 2 years of clinical nursing experience.
3. Strong communication and interpersonal skills.
4. Proficiency in using electronic health records (EHR) and other healthcare technologies.
5. Ability to work independently and manage time effectively.
6. Compassionate and patient-centered approach to care.
7. High-speed internet connection and a quiet, private workspace.
Responsibilities
1. Conduct telephonic assessments to identify patient needs and provide appropriate health advice.
2. Educate patients on managing chronic conditions and promoting healthy lifestyles.
3. Coordinate care with other healthcare professionals and connect patients with necessary resources.
4. Document patient interactions and maintain accurate records in the EHR system.
5. Monitor patient progress and follow up as needed to ensure adherence to care plans.
6. Provide emotional support and guidance to patients and their families.
7. Participate in ongoing training and professional development to stay current with best practices in telehealth.

Educational Qualifications
– Bachelor of Science in Nursing (BSN) preferred.
– Current and valid RN license.
Experience
