We are seeking a dedicated and experienced Remote Nursing Coordinator to join our team at CVS Health. As a Remote Nursing Coordinator, you will play a vital role in coordinating and managing patient care remotely while ensuring a high level of service and compliance with healthcare protocols.

Job Description:

As a Remote Nursing Coordinator, you will be responsible for overseeing and managing patient care plans, collaborating with healthcare providers, and ensuring effective communication between all parties involved in patient care. You will work closely with doctors, nurses, and other healthcare professionals to create comprehensive care plans tailored to the individual needs of each patient.

You will also play a critical role in educating patients and their families about their health conditions and treatment options, ensuring they have the information and support they need to make informed decisions about their care. Your ability to work independently, manage multiple tasks, and maintain detailed records will be key to your success in this role.

Requirements:

• Active and unrestricted Registered Nurse (RN) license.
• Minimum of 3 years of clinical nursing experience.
• Proficiency in electronic health record (EHR) systems.
• Strong organizational and time management skills.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Proficiency in Microsoft Office Suite (Word, Excel, Outlook).

Responsibilities:

• Coordinate and manage patient care plans remotely.
• Collaborate with healthcare providers to develop comprehensive care plans.
• Ensure effective communication between patients, families, and healthcare providers.
• Educate patients and their families about health conditions and treatment options.
• Monitor patient progress and adjust care plans as necessary.
• Maintain detailed records of patient interactions and care plans.
• Participate in ongoing training and development programs to stay current with best practices.

Educational Qualifications:

• Bachelor? degree in Nursing (BSN) preferred.
• Active and unrestricted Registered Nurse (RN) license.

Experience:

• Minimum of 3 years of clinical nursing experience.
• Experience in care coordination or case management preferred.
