Description

CVS Health is seeking a dedicated and compassionate Remote Chronic Disease Nurse to join our innovative team. This position involves working remotely, providing care and support to patients with chronic diseases, helping manage their conditions, and improving their overall quality of life. As a part-time role, you will work under 4 hours a day, offering the flexibility to balance work with other life commitments. The ideal candidate will be someone with a strong nursing background, excellent communication skills, and a passion for patient care.

Requirements

1. Active RN license in the state of residence.

2. Minimum of 2 years of clinical nursing experience.

3. Experience in managing chronic diseases such as diabetes, hypertension, and COPD.

4. Strong communication and interpersonal skills.

5. Proficiency in using electronic health records (EHR) and telehealth platforms.

6. Ability to work independently and manage time effectively.

7. Reliable internet connection and a quiet workspace.

Responsibilities

1. Provide telehealth consultations to patients with chronic diseases.

2. Develop and implement personalized care plans.

3. Monitor patient progress and adjust care plans as needed.

4. Educate patients and their families about managing chronic conditions.

5. Coordinate with other healthcare professionals to ensure comprehensive care.

6. Document all patient interactions and updates in the EHR.

7. Participate in continuous professional development and training programs.

Educational Qualifications

– Bachelor of Science in Nursing (BSN) preferred, Associate Degree in Nursing (ADN) required.

Experience
