ACT Now Prevention Case Manager (part time 18.5 hrs p/w FTC 12 months)

The Youth Justice Team is a busy, supportive and passionate team of people who deliver excellent support to children who have offended or who are at risk of offending. A Her Majesty?s Inspectorate of Probation inspection that took place in November 2023 rated Bracknell Forest?s Youth Justice Services as ?Good? with a... number of outstanding areas of practice.
The Team is multi-disciplinary as it includes police, health and speech and language colleagues and it also benefits from strong working relationships with Children?s Social Care, Education and Early Help services. All these services working together and with children in our cohort, means that we have a dedicated team around each child to maximise their life chances.
Visible, invested and experienced leadership is in place to ensure that the team are supported with any challenges they face ? a number of people in the team have worked for Bracknell Forest Council for over 20 years!
A normal day may include a visit to a child and their family to undertake an assessment, liaising with team specialists on a speech and language assessment, exploring activities and interventions that the child is interested in and would benefit from, writing up case notes, visiting a child in custody, undertaking training and attending partnership meetings.
We have also just secured our very own building which is central, accessible and child-friendly.
Key responsibilities...
The aim of the Act (Achieving Change Together) Now project is to reduce violence in our communities by offering early intervention and support to children arrested for knife-related and violent offences. It aims to divert them away from the criminal justice system and make communities safer. The programme has Child First principles at its core. We believe in rehabilitation and empowerment, providing support that encourages positive change and community reintegration.
In this role, you will work closely with children who are on the periphery of youth offending and who need support to address things in their life that are proving complex and challenging.
Key responsibilities will include:
? Case Management: Develop and implement child-centred plans, monitor progress, and adjust strategies as needed.
? Advocacy: Serve as an advocate for a child, connecting them with necessary services, resources, and support systems.
? Collaboration: Work with families, professionals, schools, and community organisations to co-ordinate services and support.
? Crisis Intervention: Provide immediate support in some crisis situations and help the child develop coping strategies.
? Documentation: Maintain accurate and timely records of interactions, case progress, and outcomes.
