Primary Duties and Responsibilities: As a Patient Care Coordinator, your primary responsibility is to monitor and manage your assigned patient population, delivering personalized healthcare via phone-based communication and electronic messaging. You'll conduct structured health assessments, adhere to patient care plans, and meticulously record all interactions electronically, including setting reminder follow-ups. Additionally, you'll create electronic alert escalations and promptly notify physicians and nurses of any changes in patient status, following pre-established protocols. Requirements: ? Preferred background in healthcare. ? Proficient in computer usage, software, and technology. ? Comfortable spending the entire workday on the phone and computer, making 30-70 outbound calls daily. ? Ability to work remotely and independently, ensuring a noise-free and private environment for phone conversations. ? Availability for 20 hours per week, with flexible shifts between Monday and Friday, 8 am to 6 pm. Perform structured health assessments and follow patient care plans to support patient care, while recording all interactions electronically (entering notes, setting reminder follow-ups) Create electronic alert escalations and otherwise notify physicians and nurses based on pre-established rules regarding changes in patient status. Requirements: Background in Healthcare preferred Skilled in using computers, software, and technology Happy to spend ALL DAY on the phone and in front of a computer -- making 30 - 70 outbound phone calls PER DAY Provide your own computer and internet access. Strong verbal, phone, written and electronic communication skills. Ability to work remotely and independently (i.e. outside of an office). Ability to work in an environment that is free of outside noise (so others can hear you well) and private from others hearing your conversations on the phone (so your conversations are private) 20 hours per week with varying shifts either Monday - Friday,8am - 6pm (The role does offer some flexibility to suit your needs). Willingness to make yourself available to the needs of your patients outside of typical office hours, including weekends, will be required. Monthly targets must be met and/or exceeded without fail. Remote Patient Monitoring, Chronic Care Management & Principle Care Management experience preferred but not required.
