The candidate selected to fill the Care Management Supervisor position will be responsible for leading the non-clinical staff with addressing the medical and psychosocial components of members needs; and providing resources and linkages to government programs, social services, and community resources.

The Care Management Supervisor will be assigned caseloads (no more than 15 members) and will oversee and support staff via supervision and 1:1 meetings. The Supervisor will provide extra support for the team members and other department support needs which may include:

supervising Adult Care Management (CM) by monitoring and measuring productivity;

ensuring timely staff communication with members, providers, and community-based organizations; provide training to staff; resolve staff issues; and identify areas for program improvement.

Supervisory Responsibilities:

Lead, mentor, and supervise CM staff; goal setting and performance appraisal; and time off request and productivity. In collaboration with management, perform annual reviews and necessary disciplinary actions: In collaboration with Director of Care Management - train and mentor new team members. Assist and coordinate staff assignments and coverages to ensure staff is performing member-centered assessments/reassessments and care plans in a timely manner . Participate in internal and external audits. Identify and address workflow process inefficiencies and work with management to resolve. Collaborate with intra- and interdepartmental colleagues as needed, or as directed by department Director. Establish and sustain relationships with community systems to facilitate linkage, referral, advocacy, and follow up with the focus on attaining care plan goals as directed by department Manager. Assist with member concerns and grievances.

Skills/Knowledge/Abilities Required:

Ability to clearly and effectively communicate and negotiate with members and their families and/or support system(s), physicians, and other health care providers.

Self-motivated to seek knowledge in order to improve personal and professional performance.

Ability to work effectively in a cross-functional team environment.

Strong computer literacy and skills.

Excellent verbal communication and writing skills.

Regular attendance is essential to perform this job.

Preferred:

Master's degree

Language: Spanish (Strongly Preferred)

Work Location: In person for a 90-day onboarding period and remote after onboarding is completed successfully
