Case Manager

Pay Rate: $24-$26/hr.

Contract: Temp to Hire

Schedule: Monday-Friday 8:30AM-5:30PM

Fluent in English and one of the Santa Clara Country threshold languages (Spanish, Tagalog, Vietnamese, Mandarin, Cantonese) strongly preferred.

Primary Responsibilities: ? Engages and enrolls community members eligible for ECM per care plan protocols ? Assesses client need areas ? Develops an individualized care plan with each client that is updated as needed ? Coordinates care for clients with a multidisciplinary clinical team and outside agencies. ? Facilitates referral linkages to community resources and performs case management services. ? Assist clients with the development of social and community support systems. ? Develops effective working relationships with agencies, organizations and providers to advocate for clients. ? Documents and maintains clinical records per appropriate standards and regulations. ? Conducts outreach in diverse community settings. ? Participate in clinical supervision, consultations, and trainings. ? Provide translation services as needed. ? Complete other duties and related projects as assigned.

Required Qualifications: ? Bachelor?s degree in psychology or a related field, or equivalent training and experience that would provide the ability to perform the duties described above. ? One year or more of case management or related experience. ? Fluent in English and one of the Santa Clara Country threshold languages (Spanish, Tagalog, Vietnamese, Mandarin, Cantonese) preferred ? Trauma informed training preferred

Preferred Qualifications: ? Familiarity with community-based resources. ? Excellent problem-solving and organizational skills. ? Understanding of cultural issues, community resources and family dynamics. ? Ability to communicate well with people of diverse cultural, professional, and experiential background. ? Ability to establish and maintain effective work relationships as part of a multidisciplinary team.

Pay Details: $24.00 to $26.00 per hour
