Your Role
? Provide monthly calls and support to patients enrolled in the CCM program.
? Utilize your nursing skills to educate patients and their families.
? Monitor patients progress and work with the physicians to ensure quality patient care.

Qualifications
? Active LPN license (CALIFORNIA)
? Experience in geriatrics or primary care
? Ability to sit and talk on the phone for long periods of time
? System: We require you to have a Windows i5 (or newer) processor, and to have Windows Pro (not Home). This can be a laptop or desktop.
? You live in any of the following states: NV, NM, AZ, CO, UT, WA, OR

The Pay
? The California LPN pay ranges from $28.85 - $30.27 per hour
? The pay is a base pay of $24.00/hr plus a bonus of $2.00 per call. On average our Care Coordinators complete between 16-21 calls per day. High performers may be eligible for a bonus up to $1000 per quarter.
