Key Responsibilities
? Conduct thorough assessments to identify clinical and psychosocial needs of individuals.
? Address health and safety concerns, including social determinants of health.
? Collaborate with a multidisciplinary team to create a personalized Life Plan.
? Regularly monitor and update Life Plans to ensure optimal care.
? Facilitate access to medical and community resources to promote health and wellness.
? Coordinate referrals for clinical, long-term, and social support services.
? Engage families and natural support systems to enhance care coordination.
? Ensure that services provided are culturally and linguistically appropriate.
? Advocate for individuals to foster self-advocacy and independence.
? Utilize Health Information Technology for efficient documentation and communication.
? Maintain compliance with HIPAA, Medicaid, and all relevant regulatory standards.
? Keep precise and detailed records of services rendered.
? Participate in ongoing training and professional development to enhance skills.

Required Skills
? Proven experience working with individuals with intellectual and developmental disabilities.
? Strong organizational and time management capabilities.
? Ability to operate independently and effectively manage crisis situations.
? Basic proficiency in technology for documentation and communication.
? Valid driver?s license.

Qualifications
? A Bachelor's degree coupled with a minimum of two years of relevant experience, OR
? A Registered Nurse (RN) license with at least two years of pertinent experience (not restricted to case management/service coordination), OR
? A Master's degree with one year of relevant experience.
