As a Chronic Care Nurse, you will play a crucial role in providing patient-centered care to individuals with chronic conditions, helping them manage their health from the comfort of their homes. **Responsibilities**

1. **Patient Assessment:** Conduct thorough assessments of patients with chronic conditions to develop personalized care plans.

2. **Care Coordination:** Coordinate with interdisciplinary teams to ensure comprehensive care and follow-up for patients.

3. **Patient Education:** Educate patients and their families about chronic disease management, medication adherence, and lifestyle modifications.

4. **Telehealth Services:** Provide telehealth consultations, monitoring patient progress, and making necessary adjustments to care plans.

5. **Documentation:** Maintain accurate and timely documentation of patient interactions, care plans, and outcomes.

6. **Support Services:** Offer emotional support and counseling to patients and families dealing with chronic illnesses.

7. **Quality Improvement:** Participate in ongoing quality improvement initiatives to enhance patient care and outcomes.

Requirements

1. **Licensure:** Active Registered Nurse (RN) license in good standing.

2. **Experience:** Minimum of 2 years of experience in chronic disease management or a related field.

3. **Technology:** Proficient in using telehealth platforms and electronic health record (EHR) systems.

4. **Communication:** Excellent verbal and written communication skills.

5. **Problem-Solving:** Strong critical thinking and problem-solving abilities.

6. **Patient Advocacy:** Commitment to advocating for patients’ health and well-being.

7. **Flexibility:** Ability to work flexible hours to meet patient needs, including occasional weekends or evenings.

Educational Qualifications

– **Degree:** Bachelor of Science in Nursing (BSN) preferred; Associate Degree in Nursing (ADN) considered with relevant experience.

– **Certifications:** BLS (Basic Life Support) required; additional certifications in chronic care or telehealth nursing are a plus.

Experience

– **Chronic Disease Management:** At least 2 years of experience working with patients with chronic conditions such as diabetes, hypertension, COPD, etc.

– **Telehealth:** Previous experience providing telehealth services is highly desirable.
