This remote position allows you to work from the comfort of your home while making a significant impact in the lives of individuals in the Beverly, Gloucester, Lynn, Danvers, Salem, and... Peabody areas.

Key Responsibilities
? Evaluate individual needs and devise customized care plans.
? Facilitate connections to vital resources, including social services, community support programs, and financial assistance.
? Advocate for members to ensure their preferences and requirements are prioritized by the healthcare team.
? Maintain clear and continuous communication with all relevant stakeholders to support effective care coordination.

? Willingness to travel within the designated region to conduct visits with members.
