Job Description

CVS Health is seeking a dedicated and experienced Remote Clinical Nurse Advisor to join our dynamic clinical team. As a Clinical Nurse Advisor, you will play a pivotal role in providing professional and comprehensive telephonic nursing services. You will engage with patients to evaluate their health needs, provide education, and coordinate care to ensure proper treatment and follow-up. Requirements

– Active Registered Nurse license in the state of residence

– Minimum of 2 years of nursing experience, preferably in a clinical or acute care setting

– Strong clinical assessment and critical thinking skills

– Excellent communication and interpersonal skills

– Ability to work independently and manage time effectively

– Proficiency in electronic health records (EHR) systems

– Reliable internet connection and a quiet, professional workspace

Responsibilities

– Conduct telephonic assessments and provide clinical advice to patients

– Develop, implement, and evaluate individualized care plans

– Educate patients on disease management, medication adherence, and lifestyle modifications

– Coordinate care with other healthcare professionals and services

– Monitor patient progress and adjust care plans as necessary

– Document all patient interactions and care plans accurately and promptly

– Participate in ongoing training and professional development initiatives

Educational Qualifications

– Bachelor of Science in Nursing (BSN) preferred, Associate Degree in Nursing (ADN) accepted with relevant experience

Experience

– Minimum of 2 years of experience in a clinical or acute care setting

– Prior experience in telehealth or remote nursing roles is a plus
