Requirements

– Active Registered Nurse (RN) license in the state of residence

– Minimum of 2 years of clinical nursing experience

– Strong communication and interpersonal skills

– Proficiency in using electronic health records (EHR) and telehealth platforms

– Ability to work independently and manage time effectively

– Excellent problem-solving skills and attention to detail

– High level of empathy and patient-centered care approach

Responsibilities

– Conduct telephonic and digital assessments to identify patient needs

– Develop and implement individualized care plans

– Educate patients about their medical conditions and treatment options

– Coordinate with healthcare providers, pharmacies, and other resources to ensure comprehensive care

– Monitor patient progress and adjust care plans as needed

– Provide emotional support and resource guidance to patients and their families

– Document all patient interactions and maintain accurate records in the EHR system

Educational Qualifications

– Bachelor of Science in Nursing (BSN) preferred

Experience
