As a Remote Nurse Clinical Coordinator, you will work closely with patients, healthcare providers, and multidisciplinary teams to facilitate seamless care transitions, manage patient cases, and enhance the overall patient experience. You will utilize your clinical expertise and strong organizational skills to develop and implement care plans, monitor patient progress, and provide education and support to patients and their families.

Requirements

• Active Registered Nurse (RN) license in the state of practice


• Minimum of 3 years of clinical nursing experience


• Previous experience in care coordination or case management is preferred


• Strong communication and interpersonal skills


• Ability to work independently and as part of a team


• Proficiency in electronic health records (EHR) and other clinical software


• Flexibility to work remotely and adapt to changing healthcare needs

Responsibilities

• Coordinate and manage the care of patients with complex healthcare needs


• Develop and implement individualized care plans


• Monitor patient progress and adjust care plans as necessary


• Provide education and support to patients and their families


• Collaborate with healthcare providers and multidisciplinary teams


• Ensure compliance with clinical standards and regulatory requirements


• Document patient interactions and care plans accurately and timely

Educational Qualifications

• Bachelor of Science in Nursing (BSN) preferred


• Active and unrestricted Registered Nurse (RN) license

Experience

• Minimum of 3 years of clinical nursing experience


• Experience in care coordination or case management is preferred
