CVS Health is seeking a dedicated and compassionate Remote Nurse Navigator to join our dynamic healthcare team. Requirements

– Active Registered Nurse (RN) license in the state of residence

– Minimum of 3 years of clinical nursing experience

– Strong knowledge of healthcare systems and patient care coordination

– Proficiency in electronic medical records (EMR) software

– Excellent verbal and written communication skills

– Ability to work independently and manage multiple priorities

– Compassionate, patient-centered approach to care

Responsibilities

– Provide personalized care coordination for patients, ensuring they receive necessary medical services

– Assist patients in understanding their medical conditions, treatment options, and care plans

– Act as a liaison between patients, caregivers, and healthcare providers to facilitate effective communication

– Monitor patient progress and adjust care plans as needed to achieve optimal outcomes

– Educate patients and caregivers on managing chronic conditions and navigating the healthcare system

– Utilize electronic medical records (EMR) to document patient interactions and track care plans

– Participate in ongoing training and professional development opportunities

Educational Qualifications

– Bachelor of Science in Nursing (BSN) preferred

– Active Registered Nurse (RN) license in good standing

Experience

– Minimum of 3 years of clinical nursing experience, preferably in care coordination or case management
