Job Description

CVS Health is seeking a dedicated and experienced Telehealth Chronic Care Nurse to join our dynamic team. As a Telehealth Chronic Care Nurse, you will be responsible for providing comprehensive care management and support to patients, ensuring they receive the necessary education and resources to manage their health effectively.

You will utilize telehealth technology to conduct virtual consultations, monitor patient progress, and adjust care plans as needed. Requirements

– Registered Nurse (RN) with an active, unrestricted license in the state of practice.

– Minimum of 3 years of clinical experience, preferably in chronic care management.

– Proficient in telehealth platforms and electronic health records (EHR).

– Excellent communication and interpersonal skills.

– Strong organizational and time-management abilities.

– Ability to work independently and as part of a team.

– Compassionate and patient-focused approach to care.

Responsibilities

– Conduct virtual consultations with patients to assess their health status and needs.

– Develop and implement individualized care plans for patients with chronic conditions.

– Monitor patient progress and adjust care plans as necessary.

– Educate patients and their families about chronic disease management and preventive care.

– Collaborate with healthcare providers to ensure comprehensive and coordinated care.

– Document patient interactions and care plans accurately in the EHR.

– Participate in ongoing training and professional development to stay current with best practices in telehealth and chronic care management.

Educational Qualifications

– Bachelor of Science in Nursing (BSN) preferred; Associate Degree in Nursing (ADN) with relevant experience will be considered.

– Certification in Chronic Care Management or Telehealth Nursing is a plus.

Experience

– At least 3 years of clinical nursing experience, with a focus on chronic care management.

– Experience in telehealth or remote patient monitoring is highly desirable.
