About the position
The Case Manager MSW is a professional role focused on providing comprehensive social work services within the emergency department, particularly through virtual assessments. This position is integral to an interdisciplinary team that is pioneering a new approach to behavioral health care. The Case Manager collaborates closely with physicians, healthcare team members, and patients and their families to ensure coordinated care across various settings during an episode of illness. This role is essential in addressing the psychosocial needs of patients and their families, providing crisis intervention, and facilitating discharge planning to ensure smooth transitions in care. In this role, the Case Manager will perform psychosocial evaluations to assess the needs of patients at risk, including those with substance abuse issues, psychiatric problems, domestic violence situations, pediatric concerns, end-stage dementia, developmental disabilities, and CVI. The Case Manager will develop individualized social work treatment plans and provide counseling to patients and their families as needed. This includes evaluating the socio-emotional status of patients and their families, providing direct services such as crisis intervention and discharge planning, and coordinating post-hospital care services. The Case Manager will also serve as a resource for patients, families, and staff, offering education on the emotional impacts of medical treatment and related topics.
Responsibilities

Requirements
? Masters degree in social work required.

? State of Michigan social work license required (LLMSW minimum).
