Job Description
CVS Health is seeking a dedicated and experienced Telephonic Triage Nurse to join our dynamic team. This role is essential in providing high-quality telephonic nursing assessment, patient education, and care coordination. You will be the first point of contact for patients seeking medical advice and support, offering timely and professional guidance to ensure optimal health outcomes. Requirements
1. Active and unrestricted Registered Nurse (RN) license.
2. Minimum of 3 years of clinical nursing experience, preferably in an acute care setting.
3. Strong telephonic communication and assessment skills.
4. Proficiency with computer systems and electronic health records.
5. Ability to work independently and make informed decisions.
6. Compassionate, patient-centered approach.
7. Excellent organizational and multitasking skills.
Responsibilities
1. Conduct telephonic assessments to determine the nature and urgency of patient health concerns.
2. Provide evidence-based medical advice and patient education.
3. Coordinate care and referrals to appropriate healthcare services and providers.
4. Document all patient interactions and care instructions accurately in the electronic health record system.
5. Monitor patient progress and follow up as needed to ensure comprehensive care.
6. Collaborate with other healthcare professionals and departments to ensure a seamless patient experience.
7. Stay current with medical guidelines and telehealth best practices.

Educational Qualifications
– Bachelor of Science in Nursing (BSN) preferred, but not required if the candidate has substantial experience.
Experience
