Description

CVS Health is seeking a dedicated and experienced Virtual Public Health Nurse to join our dynamic team. As a Virtual Public Health Nurse, you will be at the forefront of providing telehealth services to our diverse patient population. You will play a pivotal role in health education, disease prevention, and patient advocacy, ensuring accessible and quality healthcare for all. This part-time position offers the flexibility of working from home while making a significant impact on public health.

Requirements

• Registered Nurse (RN) license in the state of residence


• Minimum of 3 years of nursing experience, preferably in public health or community nursing


• Excellent communication and interpersonal skills


• Proficiency in telehealth platforms and electronic health records (EHR) systems


• Strong organizational and time management skills


• Ability to work independently and as part of a multidisciplinary team


• Compassionate, patient-focused approach to healthcare

Responsibilities

• Conduct virtual health assessments and provide patient-centered care


• Develop and implement individualized care plans


• Educate patients on disease prevention, health promotion, and self-management


• Collaborate with other healthcare professionals to coordinate patient care


• Monitor patient progress and adjust care plans as necessary


• Document patient interactions and maintain accurate health records


• Participate in community outreach initiatives and public health campaigns

Educational Qualifications

• Bachelor of Science in Nursing (BSN) or higher


• Public Health Nursing certification preferred

Experience

• Minimum of 3 years of clinical nursing experience


• Experience in public health or community health settings preferred
