Description

CVS Health is seeking a dedicated and compassionate Remote Health Coach Nurse to join our dynamic team. This position is ideal for nursing professionals who are passionate about enhancing patient care and wellness through innovative telehealth services. As a Health Coach Nurse, you will support patients in managing their health, reaching wellness goals, and navigating the healthcare system. You will work closely with a multidisciplinary team to deliver personalized health coaching and educational services remotely.

Requirements

• Valid Registered Nurse (RN) license in the state of residence


• Minimum of 3 years of clinical nursing experience


• Experience in health coaching or patient education


• Exceptional communication and interpersonal skills


• Proficient in telehealth technologies and electronic health record systems


• Ability to work independently and as part of a team


• Strong organizational and time-management skills

Responsibilities

• Provide personalized health coaching and education to patients via telehealth platforms


• Develop and implement individualized care plans to meet patients?ealth and wellness goals


• Monitor patients?rogress and adjust care plans as necessary


• Collaborate with healthcare providers to ensure coordinated and comprehensive patient care


• Support patients in understanding and managing chronic conditions


• Conduct regular follow-up sessions to track patients?rogress and address any concerns


• Maintain accurate and up-to-date patient records in electronic health systems

Educational Qualifications

• Associate or Bachelor? degree in Nursing (BSN preferred)

Experience

• At least 3 years of clinical nursing experience


• Previous experience in health coaching or patient education is highly desirable
