Job Description

As a Remote Nurse Advocate at CVS Health, you will be an integral part of our mission to provide high-quality healthcare services to our clients. Requirements

– Active RN license in the state of residence

– Minimum of 3 years of clinical experience, preferably in a home health, hospice, or case management setting

– Strong communication and interpersonal skills

– Proficiency in using electronic health records (EHR) systems

– Reliable high-speed internet connection

– Comfortable using telehealth platforms and technology

– Ability to work independently and as part of a team

Responsibilities

– Provide telephonic and virtual care to patients, offering support and guidance

– Educate patients on their health conditions and treatment plans

– Coordinate care with other healthcare providers to ensure continuity and quality of care

– Monitor patient progress and adjust care plans as necessary

– Document patient interactions and maintain accurate records in the EHR system

– Participate in ongoing training and professional development activities

– Advocate for patients to ensure they receive the best possible care

Educational Qualifications

– Bachelor of Science in Nursing (BSN) preferred

Experience

– Minimum of 3 years of clinical nursing experience
