Job Description

Requirements

1. Registered Nurse (RN) with an active license in the state of practice.

2. Minimum of 2 years of clinical nursing experience.

3. Experience in telehealth or telemedicine services preferred.

4. Strong communication and interpersonal skills.

5. Proficient with electronic health records (EHR) and telemedicine platforms.

6. Ability to work independently and as part of a team in a remote setting.

7. Strong organizational and time-management skills.

Responsibilities

1. Conduct telehealth consultations, assessments, and follow-ups with patients.

2. Develop and coordinate individualized care plans and patient education.

3. Collaborate with physicians, specialists, and other healthcare providers to ensure comprehensive patient care.

4. Monitor and manage patient health data using advanced telehealth technology.

5. Provide support and guidance to patients regarding their health concerns and treatment plans.

6. Facilitate communication between patients, caregivers, and healthcare teams.

7. Maintain accurate and timely documentation of patient interactions and care plans.

Educational Qualifications

– Bachelor of Science in Nursing (BSN) preferred.

– Registered Nurse (RN) with current licensure.

Experience

– Minimum of 2 years of clinical nursing experience.
