Requirements

• Active and unrestricted RN license in your state of residence


• Minimum of 2 years of nursing experience, preferably in a clinical setting


• Experience in case management or care coordination is a plus


• Strong communication and interpersonal skills


• Proficiency in using electronic medical records and telehealth platforms


• Ability to work independently and as part of a remote team


• Reliable internet connection and a quiet, private workspace

Responsibilities

• Conduct telephonic assessments to evaluate patients’ health status and needs


• Develop and implement personalized care plans based on patients’ health goals


• Provide education and resources to patients and caregivers on managing chronic conditions


• Coordinate with healthcare providers to ensure continuity of care


• Monitor patients’ progress and adjust care plans as needed


• Maintain accurate and timely documentation in electronic medical records


• Participate in team meetings and professional development opportunities

Educational Qualifications

• Associate’s or Bachelor’s Degree in Nursing from an accredited institution

Experience
