Description:
CVS Health is looking for a dedicated and experienced Telephonic Nurse Case Manager to join our dynamic and compassionate team. As a Telephonic Nurse Case Manager, you will be responsible for managing a case load of patients through telephonic interactions, providing comprehensive care coordination, patient education, and support to ensure optimal health outcomes. This remote position offers flexibility and the opportunity to make a meaningful impact on patients’ lives from the comfort of your own home.
Requirements:
1. Active Registered Nurse (RN) license in good standing.
2. Minimum of 3 years of clinical nursing experience.
3. Experience in case management, discharge planning, or home health.
4. Proficient in using electronic medical records (EMR) and other healthcare technology.
5. Excellent communication and interpersonal skills.
6. Ability to work independently and manage time effectively.
7. Strong critical thinking and problem-solving abilities.
Responsibilities:
1. Conduct telephonic assessments and develop individualized care plans for patients.
2. Coordinate care among healthcare providers, patients, and their families.
3. Educate patients and families on disease processes, medications, and care plans.
4. Monitor patient progress and adjust care plans as needed.
5. Advocate for patients to ensure they receive necessary services and resources.
6. Document all interactions and care plans accurately in the EMR.
7. Participate in team meetings and collaborate with other healthcare professionals.

Educational Qualifications:
– Bachelor? degree in Nursing (BSN) preferred.
– Associate degree in Nursing (ADN) with relevant experience will be considered.
Experience:
– Minimum of 3 years of clinical nursing experience.
– Previous experience in case management, discharge planning, or home health is highly desirable.
