Job Description

In this role, you will provide end-of-life care and support to patients and their families, ensuring comfort and dignity during their final stages.

The Remote Hospice Nurse will work from home, offering telephonic and video consultations, coordinating care plans, and collaborating with multidisciplinary teams to ensure the best possible outcomes for our patients. This position requires a combination of clinical expertise, emotional intelligence, and strong communication skills to manage the physical and emotional needs of hospice patients.

Requirements

1. Current and unrestricted RN license in the state of practice.

2. Minimum of 2 years of experience in hospice or palliative care.

3. Proficient in using electronic medical records (EMRs) and telehealth platforms.

4. Strong understanding of pain management and end-of-life care principles.

5. Excellent communication and interpersonal skills.

6. Ability to work independently and as part of a multidisciplinary team.

7. Reliable high-speed internet connection and a quiet, private work environment.

Responsibilities

1. Provide telephonic and video consultations to hospice patients and their families.

2. Develop and implement individualized care plans in collaboration with the healthcare team.

3. Monitor patients’ conditions, manage symptoms, and adjust care plans as needed.

4. Educate patients and families on disease progression, symptom management, and end-of-life care options.

5. Coordinate with physicians, social workers, and other healthcare professionals to ensure comprehensive care.

6. Document patient interactions and care plans accurately and promptly in the EMR system.

7. Participate in regular team meetings and continuing education to stay updated on best practices in hospice care.

Educational Qualifications

– Bachelor of Science in Nursing (BSN) preferred

– Hospice and Palliative Nurses Association (HPNA) certification is a plus

Experience

– Minimum of 2 years of experience in hospice or palliative care

– Experience with telehealth platforms and electronic medical records
