Description

CVS Health is looking for a dedicated and compassionate Telehealth Clinical Nurse to join our dynamic team. As a Telehealth Clinical Nurse, you will play a vital role in providing high-quality healthcare services remotely, ensuring our patients receive timely and effective care. This position is fully remote, allowing you to work from the comfort of your home while making a significant impact on patient health outcomes.

The ideal candidate will possess strong clinical skills, excellent communication abilities, and a passion for patient-centered care. You will work collaboratively with other healthcare professionals to deliver personalized care plans, answer patient inquiries, and provide guidance on managing various health conditions.

Requirements

• Active Registered Nurse (RN) license in the state of practice


• Minimum of 2 years clinical nursing experience


• Experience in telehealth or remote patient monitoring preferred


• Strong communication and interpersonal skills


• Proficiency with electronic health records (EHR) systems


• Ability to work independently and manage time effectively


• Comfortable with technology and virtual communication tools

Responsibilities

• Conduct virtual consultations with patients to assess health needs and provide appropriate medical advice


• Develop and implement personalized care plans in collaboration with healthcare team members


• Monitor patient progress and adjust care plans as necessary


• Provide patient education on managing chronic conditions, medications, and lifestyle changes


• Document patient interactions and maintain accurate medical records


• Coordinate with other healthcare providers to ensure continuity of care


• Participate in ongoing training and professional development activities

Educational Qualifications

• Bachelor of Science in Nursing (BSN) preferred


• Active Registered Nurse (RN) license

Experience

• Minimum of 2 years clinical nursing experience


• Experience in telehealth or remote patient monitoring preferred
