Requirements


1. Active RN licensure in the state of practice.


2. Minimum of 3 years of clinical experience.


3. Strong telephonic communication skills.


4. Proficiency in using electronic health records (EHR) and other healthcare technology.


5. Excellent problem-solving and critical-thinking abilities.


6. Ability to work independently and manage time effectively.


7. Compassionate and patient-centric approach to healthcare.


Responsibilities


1. Conduct telephonic assessments to understand member needs and develop individualized care plans.


2. Coordinate and monitor care plans, ensuring members receive appropriate and timely services.


3. Provide education and guidance to members on managing their health conditions.


4. Collaborate with other healthcare professionals, including social workers, physicians, and therapists.


5. Monitor member progress and adjust care plans as necessary.


6. Document all interactions and care plans accurately in the EHR system.


7. Advocate for members to ensure they receive the best possible care and resources.

Educational Qualifications


– Bachelor? Degree in Nursing (BSN) preferred.


– Active and unrestricted RN license.


Experience


– Minimum of 3 years of clinical nursing experience, preferably in case management or related field.


– Experience in telephonic case management is a plus.
